On An Average Day You Will:
(includes, but not limited to)
- Provide palliative and advanced illness care in collaboration with physicians and the interdisciplinary care team
- Perform patient assessments, develop treatment plans, manage symptoms, and coordinate ongoing care
- Evaluate hospice eligibility and provide clinical recommendations for certification and recertification
- Prescribe medications, order treatments, and coordinate referrals and consultations within scope of practice
- Collaborate with physicians, nurses, and other healthcare professionals to ensure high-quality patient care
- Educate and counsel patients, families, and caregivers regarding disease management, goals of care, and end-of-life planning
- Maintain accurate, timely, and compliant clinical documentation and patient records
- Participate in interdisciplinary team meetings and care planning activities
- Serve as a clinical resource and educator for staff, providers, and community partners
- Develop and maintain relationships with referral sources and healthcare partners to support program growth
- Support quality improvement initiatives and ensure compliance with hospice regulations and standards
- Maintain patient confidentiality and adherence to all regulatory and organizational requirements
- Perform other duties and special projects as assigned
What You'll Bring to The Team:
- Graduate of an accredited Nurse Practitioner program
- Current Nurse Practitioner licensure and advanced practice certification as required by state regulations
- Master’s degree preferred
- Minimum one (1) year of Nurse Practitioner experience and one (1) year of palliative care experience preferred
- Certification in palliative care, hospice, geriatrics, or a related specialty preferred
- Strong clinical assessment, communication, problem-solving, and documentation skills
- Ability to collaborate effectively with physicians, interdisciplinary teams, patients, and families
- Proficiency with electronic medical records and healthcare software systems
- Flexible work schedule and commitment to ongoing professional development
