The Hospice Transition Specialist provides palliative care for patients aging, chronic, complex, and/or end of life conditions while building and maintaining relationships with key referral sources. This position serves as a clinical and educational team lead for patients.

An Average Day: 
(Includes, but not limited to)

  • Facilitate initiatives that promote hospice access in the assigned hospital(s)
  • Ensure that the educational needs of the hospital staff and physicians as it relates to hospice care and services are assessed and met on an ongoing basis
  • Continuously conduct education needs assessments and develop an associated plan designed to meet the needs of the assigned hospital(s)
  • Establish and monitor customer, payer and patient perceptions of Bristol Hospice as a high-quality provider of hospice services in the assigned hospital(s)
  • Facilitate communication with agency and referral sources as needed to promote the continuum of care
  • Maintain comprehensive working knowledge in the field of community educational resources and share information with appropriate organizational personnel
  • Facilitate the hospice referral process
  • Serve as a resource to review and consult available patient information related to the case, to determine hospice care needs and criteria
  • Coordinate and communicate with hospice agency leadership and referral sources to promote care coordination and a timely admission process
  • Confer with the Medical Director or Attending Physician regarding patient hospice determination criteria
  • Obtain physician orders and Certification of Terminal Illness, as well as other admission documentation as required
  • Complete hospice consults/evaluations and admissions
  • Ensure coordination and communication with referral sources and hospice agency leadership
  • Complete an initial assessment of patient and family to determine hospice needs
  • Facilitate and complete admissions for referrals, as indicated
  • Provide a complete physical assessment and history of current and previous illness of referral
  • Other duties as assigned

Requirements:

  • Must be a graduate of an accredited school of nursing
  • Must have one (1) year experience as a professional nurse within the last three (3) years or have a bachelor’s degree in nursing from a program accredited by the National League of Nursing
  • Must have a current RN license in the state
